As healthcare providers, we spend a significant portion of our professional lives in the "thick" of documentation. We are trained to be thorough, writing exhaustive, evidence-based evaluations that check every clinical and insurance box required.
But there is often a massive disconnect between that twenty-page report and the moment a patient actually starts treatment, especially if they are being handed off to another discipline. When information is buried in jargon and length, the "mental load" on the next provider increases, and the clarity for the patient decreases. To move toward a more streamlined practice, we need a bridge.

We write long reports for compliance, but they rarely facilitate daily action. When we hand over a 15-page evaluation, are we giving our patients a roadmap or a textbook? And when we hand off to another discipline, are we giving them a patient or a pile of data?

The solution is a simple, systematic change: The Treatment At-A-Glance Document. This is a one-page roadmap created immediately following an evaluation. It doesn’t replace your formal report, but it serves as the functional, living document for everyone involved.

To keep this systemized across your practice, your summary should focus on these four essential components:
Individual Strengths & Assets: What is the patient bringing to the table? Focus on internal resources and motivations to set a positive tone for the entire team.
Supports for Participation: What environmental or sensory supports are non-negotiable for success right now? (e.g., visual timers, quiet environments, or physical assists).
Top 1-3 Functional Goals: Write these in plain, impactful language. If a caregiver reads these, they should immediately know what "success" looks like this month.
The Path Forward: A shared commitment including recommended frequency and 1–3 high-impact, low-friction strategies for home.
